Scenario 80,000 F0 in HCMC

Maybe having to treat up to 80,000 people with Covid-19 in the near future, Ho Chi Minh City plans to prepare more hospitals – mobilized or requisitioned from private hospitals.

In the context that the number of Covid-19 cases continues to increase, correspondingly the number of serious and critical patients will also increase, the Department of Health of Ho Chi Minh City changed the treatment model from “4-storey tower” to 5 floors, on 22/02. 7. Each floor will take advantage of existing infrastructure to convert functions into facilities dedicated to receiving and treating Covid-19, corresponding to incremental F0 scenarios.

In the case of Ho Chi Minh City with 80,000 F0, first floor will receive 40,000 people (50% F0) in isolated areas of districts, districts and Thu Duc city. 2nd Floor prepare 21,600 beds (27% F0) to treat patients with symptoms and underlying diseases, at field hospitals. Floor 3 including 8,000 beds (10% F0), treating symptomatic cases. 4th floor including 6,400 beds (8% F0) in hospitals treating people with severe comorbidities requiring specialized intervention. 5th floor is a Covid-19 resuscitation hospital with 4,000 beds (5% F0).

To meet the number of beds as in the scenario, the city is increasing capacity and expanding Covid-19 treatment and reception facilities. In which, hospitals provide basic treatment, belonging to floor 3 (8,000 F0), mainly converted from district hospitals.

This group currently includes 8 hospitals, namely Can Gio, Cu Chi, Binh Chanh, Eastern Military Medical Center, Le Van Viet, Hoc Mon, Go Vap and Saigon General Hospital; with a total of 3,315 beds, 606 doctors and 1,008 nurses involved in the treatment.

It is expected that the Hospitals of Dermatology, Rehabilitation and Treatment of Occupational Diseases, Post Office, Transport, Orthopedics and Rehabilitation and some other district hospitals will be converted into function ( about 4,120 beds).

In addition, to offset the number of treatment beds as in the scenario, private hospitals can be mobilized and requisitioned when needed with 9,659 beds, including: Hoan My Saigon General Hospital, Trieu An, International City, National Saigon South, Van Hanh, Hoan My Thu Duc, Xuyen A.

4th floor Currently there are 11 hospitals, with a total of 4,041 beds, including: Pham Ngoc Thach, Trung Vuong, Thu Duc, Thu Duc City, City Children, Children 2, Nguyen Trai, Nguyen Tri Phuong, An Binh, Tu Du , Mental. A total of 578 specialists and 1,202 nurses are participating in the treatment.

In addition to increasing the capacity of the above hospitals, the city will bring a series of other hospitals to participate in treatment, bringing the total number of 4 bunk beds to 6,631, including: Hospital of University of Medicine and Pharmacy of Ho Chi Minh City, Hung Vuong, Binh Dan, Gia An 115, FV, Tim Tam Duc, Vinmec Central Park.

Currently, Ho Chi Minh City has 4 Covid-19 resuscitation hospitals under 5th floor with 1,800 beds, including: Cho Ray, Tropical Diseases, Covid-19 Resuscitation, Military Medical 175. These facilities are fully equipped with modern resuscitation facilities, tasked with intensive resuscitation of critical F0s. . It is expected that Gia Dinh People’s Hospital and People’s 115 will participate in the treatment of patients on this floor.

The human resources on the 5th floor are emergency specialists or general practitioners with short-term training in emergency and resuscitation, capable of deploying specialized techniques such as high-function ventilation, dialysis. continuity, ECMO, shock resuscitation… Nursing care nurses with college degrees or higher, with experience in emergency resuscitation.

The common difficulty of 5th floor hospitals is the lack of manpower for emergency and resuscitation specialists according to the number of beds assigned. Accordingly, Ho Chi Minh City is increasing aid resources from provinces and cities mobilized by the Ministry of Health, promoting short-term thematic training on severe and critical F0 resuscitation.

The Department of Health of Ho Chi Minh City arranges at least one doctor, two nurses to take care of and monitor each 50-100 F0 on the 1st floor, each 25 F0 on the 2nd floor, each 20 F0 on the 3rd floor, each 10 F0 on the 4th floor. and every 5 F0 on the 5th floor.

According to the HCM City Center for Disease Control, on the morning of 24/7, HCM City hospitals treated 37,407 positive patients, including positive rapid test PCR test results. Of these, 619 critically ill patients are on mechanical ventilation and 12 have ECMO interventions. To date, the city has recorded 496 deaths.

During the 4th outbreak, Ho Chi Minh City repeatedly changed its anti-epidemic scenario and model. In May, when only 3 cases were recorded, Ho Chi Minh City developed 3 medical response scenarios, in which the most serious case was 5,000 cases. Very soon, the number of infections in the city far exceeded this situation. The health sector applies the 3-storey tower strategy to treatment, preparing a scenario of 50,000 treatment beds. After half a month, the 4-storey tower model was replaced before the number of patients increased by nearly 500%.
