Arriving late to the hospital, the boy suffered from necrotic testicular torsion

HanoiAn 11-year-old boy, with scrotal pain, did not go to the hospital for two days due to fear of contracting Covid-19, when the doctor examined, the torsion testicle was necrotic and had to be removed.

Doctor Nguyen Dinh Lien, Head of the Department of Renal Surgery, Urology and Andrology, Hospital E, on December 7, said that the baby had testicular torsion pain, came to the hospital in a painful condition, the scrotum area was swollen, lost wrinkles, cords. Multi-loop spermatids lose signal from the feeding vessels.

Doctor Lien said that the patient was hospitalized late, missed the golden time for treatment, leading to necrotic testicles, which had to be removed. “This is very regrettable,” the doctor said, adding that the removal of one testicle will affect fertility due to a lack of male hormones.

The golden time to treat testicular torsion is in the first 6 hours from the time of pain. If intervention before 6 hours, 100% of patients can save testicles. Going to the hospital in about 6-12 hours, the chance of saving the testicle is 50%, in 12-24 hours, it is only 20%. Going to the hospital after 24 hours usually won’t save the testicle.

Testicular torsion is a phenomenon in which the spermatic cord is twisted around its axis, blocking the blood vessels supplying the testicle, leading to edema, congestion, and testicular necrosis. Testicular torsion can occur at any age, of which 50% are aged 16-21. The cause is usually an abnormality of the scrotal ligament. When torsion persists for several hours, the patient may have fever and leukocytosis.

The symptom of testicular torsion is pain. Patients are often awakened from sleep by a sharp pain in the scrotum, in addition to swelling in the scrotum, nausea and vomiting, a few also have urinary disturbances. The disease can be misdiagnosed with orchitis. It is one of the causes of delayed diagnosis.

Currently, the main method of treatment is surgery. Patients after orchiectomy can put a prosthetic testicle to help solve psychological problems.

Doctor Lien said that in the context of the Covid-19 epidemic, many people are concerned that they do not go to the hospital for examination but try to be in moderation or buy their own medicine. Many people also have a subjective mentality that the disease has not reached the point of having to go to the hospital for examination, skipping the golden period of treatment, making the disease worse.

The doctor advised that Covid-19 is dangerous and complicated, but the hospital has implemented epidemic prevention and control measures to ensure the safety of both patients and medical staff. Patients and their families should not be too hesitant to go to the hospital for treatment, affecting their health and lives. People can call medical facilities according to the hotline for consultation and medical examination online, avoiding unfortunate cases.
